When communicating with CritiCall Ontario via e-mail, please do not include or share any of the following information in your message:

  • Personal Health Information (PHI) of any kind including patient name, Medical Record Number(s) (MRN), Date of Birth (DOB), or any other information that identifies the specific individual; that may be used or manipulated by a reasonably foreseeable method to identify the individual; or that may be linked by a reasonably foreseeable method to other information that identifies the individual.
  • Account Passwords– Passwords should never be shared with anyone, including CritiCall Ontario.
  • Account Security Phrases (PHRS).

 

Please note that if PHI or other confidential information is required to support further investigation of the issue you have contacted us about, CritiCall Ontario will provide you with instructions on how to provide the required information to us securely.

 

Self-Serve Options:

 

If you have forgotten your password or username and are unable to sign on to the PHRS, please follow the steps below:

 

  • Forgotten Username: Click on the “Forgot your username?” link on the PHRS sign on screen, directly under the username field and follow the on-screen steps.
  • Forgotten Password: Click on the “Forgot your password?” link on the PHRS sign on screen, directly under the password field and follow the on-screen steps.

 

Inactive Accounts or Other Account Related Issues:

 

  • Please contact the PHRS Helpdesk by sending an E-Mail to phrshelpdesk@criticall.org. PHRS account related requests will receive a response within 3 business days.
